Canada Post Defined Contribution Pension (DC) enrolment form (For new members only)
New DC members – use this form to enrol in the DC pension. This form is included with all the new member DC pension kits.
Also use this form to select the contribution percentage you wish to have deducted each pay, make your DC pension investment selection and designate beneficiaries.
DC Pension Payroll Deduction Authorization form
Existing members – use this form to increase or decrease your existing DC payroll contribution percentage. The contribution percentage can only be changed once every six months. Some exceptions may apply in the case of an authorized leave of absence without pay.
Sign in to mycanadalifeatwork.com to change the amount you contribute each pay in the Voluntary Savings Plan (VSP) or contact the Canada Life Call Centre at 1-866-716-1313.
The Canada Post Spousal Voluntary Savings Plan (SVSP) enrolment form
NOTE: Only employees participating in the DC Plan can enrol in the VSP or the SVSP – Use this form to enrol in a Spousal Voluntary Savings Plan (SVSP).
Designation of revokable beneficiary/trustee appointment form
DC members – use this form to update their beneficiary information for the DCP and/or the VSP, or to appoint a Trustee or administrator if your designated pension beneficiaries are minors.
